A list of mandatory papers.

Documents required for consent to establish:

1. Applications for consent (Form I rule 4)

2. Fees for consent to establish

3. Authority letter in favor of applicant

4. Affidavit on non judicial stamp paper of Rs. 10 , duly signed and notarized

5. A feasibility report on pollution control measures covering details of raw material , products , process , water balance sources of air and water , handling and disposal of hazardous substances

6. Land allotment / land conversion order

7. Site plan / location plan

8. Conceptual building plan / factory layout showing the location of plant and green belt

9. Copy of articles and memorandum of association / partnership deed

10. Acknowledgement from DIC/ secretariat for industrial assistance

11. Copy of duly signed project report by CA

12. Source of water and copy of the receipt of application submitted for abstraction of ground water to central ground water authority / for supply of requisite quantity of water to competent authority

13. Copy of receipt of application submitted for environment clearance and copy of environmental clearance

14. Copy of mining lease

15. Approved mining plan /draft mining plan with receipt of DMG

16. Forest diversion from competent authority

17. Certificate regarding distance from abadi issued by revenue officer not below the rank of tehsildar

18. Certificate regarding existence of water body issued by office of water resource department not below the rank of assistant engineer , in original (in case of stone crusher & mineral grinding

19. Document for establishing the availability of raw material either owns mines or with agreement with other mining lease holders

20. Route map of pipe line on G.T sheet with location of nearby features viz. national park sanctuaries , ecological sensitive area

21. Registration with RREC (in case of solar / wind / hydel power plant)

Documents required for consent to operate:

1. Application for consent to operate in prescribed format , duly filled and signed by authorized authority signatory

2. Fees for consent to operate

3. Authority letter in favor of applicant / board resolution / power of attorney

4. Affidavit / undertaking of non judicial stamp paper of Rs. 10 , signed and notarized

5. C.A. certified capital investment in the project (without depreciation) as on today/end of financial year

6. Point wise evidence based compliance report of previous CTE/ CTO/CGWA

7. Analysis / monitoring report source emission /ambient/air/waste / hazardous waste from approved laboratory

8. Evidence of remittance of water cess

9. NOC for abstraction of ground water from central ground water authority commitment from competent authority for supply of requisite quantity of water

10. Copy of environmental clearance

11. Point wise evidence based compliance report of environmental clearance

12. Approved mining plan /draft mining plan with receipt of DMG (in case of mines)

13. Environment statement in prescribed format under environment protection rules 9for red category industries)

14. Record of procurement of raw material

15. Certified production figure(in case of mines / stone crusher / hot mix plant)

16. Documents for establishing availability of raw material either own mines or with agreement with other mining lease holders

17. Copy of authorization and registration under HWMR 9in case of hazardous waste processing units)

18. Copy of public liability insurance act(wherever applicable)
